Service Coordination

Consumer-driven and individually oriented service plans are based on
comprehensive, personalized application, screening, and assessment process
Continuous advocacy for consumers' right to service
Provide and coordinate necessary referrals
Ongoing maintenance and assessment of consumer service plan, including
plan modification and enhancement as needed to best serve consumers
Flexible service coordination hours to accommodate consumers' schedules
24-hour emergency on-call system

Day Habilitation
Cayuga, Tompkins Counties
One-to-one interaction and group sessions with trained staff
Builds consumers self-worth, integrity, and support
Assists with volunteer jobs, transportation to and from programs, budgeting
and purchasing, behavioral support, skill acquisition and development, health
and nutrition, exercise, structured recreation and leisure activities, and community
awareness and participation
Full or half-day service provided seven days a week
 
Residential
Cayuga, Tompkins Counties

Supervised sites provide 24-hour support by trained staff; services include
behavioral support, medication administration, assistance at medical appointments,
nursing oversight, meal preparation, help w/nutrition and Activities of Daily Living (ADLs)

At-home residential support provides trained staff support directly in home
Supportive sites include 1-2 person apartments with general staff assistance
provided three to seven times a week to maintain independent living skills

Respite available in Cayuga County only, 24-hour trained staff supervision
